What Is MediShield Life?

MediShield Life is Singapore’s mandatory national health insurance scheme. You are enrolled automatically at birth or when you become a Singapore PR. Your premiums are paid from your MediSave account — no cheque needed every month.

The scheme is run by the Ministry of Health (MOH) and covers hospitalisation and selected outpatient treatments at subsidised rates in public hospitals. It is designed for Class B2 and C ward stays in restructured hospitals.

Think of MediShield Life as the floor, not the ceiling. It caps your catastrophic hospital bills, but it does not cover private hospitals, Class A wards, or the full cost of many procedures.

When people say “AIA MediShield Life,” they are usually referring to AIA HealthShield Gold Max — AIA’s Integrated Shield Plan. It is not a separate scheme. It layers on top of MediShield Life to give you broader coverage.

MediShield Life covers the subsidised wards. AIA HealthShield Gold Max extends this to Class A wards in public hospitals or private hospitals, depending on the plan tier you choose.

You pay two sets of premiums: the base MediShield Life premium (from MediSave) and the additional AIA HealthShield premium (also from MediSave, up to withdrawal limits). AIA administers both layers together — which is why people call it “AIA MediShield Life.”

2026 Rider Changes: What Changed from 1 April 2026

This is the biggest shake-up to Singapore’s ISP landscape in years. On 26 November 2025, MOH announced new rules for all ISP riders, effective 1 April 2026.

A rider is an optional add-on to your main ISP. The old AIA riders (AIA Max VitalHealth Pro A and Pro B) used to cover your deductible and cap your co-payment at S$3,000. That meant many policyholders paid close to nothing out of pocket when hospitalised. The new rules end that.

From 1 April 2026, the old AIA Max VitalHealth riders were withdrawn. You can no longer buy them. If you already own one, it continues under your existing terms for now. If your rider was bought between 27 November 2025 and 31 March 2026, it will convert to the new structure at your first policy renewal after 1 April 2028.

Here is what the new AIA rider structure looks like:

  • Deductible: No longer covered by the rider. You pay S$1,500–S$3,500 before insurance kicks in, depending on ward class.
  • Co-payment cap: Doubled from S$3,000 to S$6,000 per year (for eligible claims — panel doctors or pre-authorised treatment).
  • Premiums: Expected to be ~30% lower on average — roughly S$600 per year in savings for a private hospital rider holder.

Yes, you pay more under the new rider. But your annual premium drops by roughly S$600. MOH data shows the average 40-year-old is hospitalised about once or twice in 20 years — so the premium savings over time often outweigh the extra co-payment.

Can You Use MediSave to Pay?

Yes. Both the deductible and co-payment under the new rider structure are MediSave-eligible, subject to prevailing withdrawal limits from the CPF Board. Your actual cash outlay can be significantly lower than the headline number.

For example, if MediSave allows S$3,500 for your procedure, your cash outlay on a S$40,000 bill drops from S$5,325 to about S$1,825. Higher than before — but far less alarming than it first sounds.

Stay on MediShield Life only if:

  • You are comfortable in Class B2 or C wards in public hospitals
  • You have no strong preference for a specific doctor or private hospital
  • You want to minimise insurance premiums

Upgrade to AIA HealthShield Gold Max B if:

  • You want Class A ward access in public hospitals — more privacy, your own specialist
