What Does MediShield Life Cover? Singapore 2026 Complete Guide

Coverage limits, exclusions, deductibles, and how to fill the gaps with an Integrated Shield Plan.

MediShield Life covers inpatient hospitalisation, day surgery, ICU stays, cancer treatments, and dialysis — but only at subsidised B2 and C-ward rates in public hospitals. It does not cover private hospitals, A-ward rooms, outpatient GP visits, or dental care. In 2026, the annual claim limit is S$150,000. Most large bills still leave you with out-of-pocket costs due to the deductible (S$3,500) and 10% co-insurance — which is why many Singaporeans add an Integrated Shield Plan on top.

TL;DR:

  • MediShield Life covers hospital stays, surgery, chemo, dialysis, and ICU — at subsidised public hospital rates only
  • You still pay a S$3,500 deductible and 10% co-insurance on every claim — your out-of-pocket costs can be significant
  • Private hospitals and A-ward rooms are not covered — you need an Integrated Shield Plan (ISP) to access these

What Is MediShield Life?

MediShield Life is Singapore’s mandatory national health insurance scheme. It is compulsory for every Singapore Citizen and Permanent Resident — you are automatically enrolled from birth and covered for life.

The scheme is administered by the CPF Board. Premiums are collected as part of your CPF contributions and vary by age. At age 30, you pay roughly S$365 per year. By age 65, that rises to around S$1,190 per year. Government subsidies and the Pioneer and Merdeka Generation packages reduce the actual amount significantly for older Singaporeans.

MediShield Life was designed to protect Singaporeans from catastrophic hospital bills — not to cover routine healthcare costs. It pays for large, unexpected expenses like surgery, long ICU stays, and cancer treatment.

As at June 2026, MediShield Life covers over 4 million Singaporeans. The annual claim limit is S$150,000. Pre-existing conditions are covered too — MediShield Life cannot exclude you.

Annual claim limit: S$150,000 — covers B2/C ward public hospitals only

What Does MediShield Life Cover?

MediShield Life covers a defined list of hospital and medical treatments. All claims must happen at approved institutions. Here is the full picture.

1. Inpatient Hospitalisation

MediShield Life pays for hospital stays at subsidised B2-ward and C-ward in public hospitals. The daily ward claimable limit is S$700 for B2 and S$360 for C. Stays of any duration are covered — there is no cap on the number of days, only on the total annual claim amount.

2. Day Surgery

Procedures that do not require an overnight stay — such as cataract surgery, colonoscopy, or minor orthopaedic procedures — are covered if performed at approved day surgery centres. This is an often-overlooked benefit.

3. Intensive Care Unit (ICU)

ICU stays are covered at up to S$1,000 per day at subsidised ward rates. If you spend three days in ICU following surgery, MediShield Life contributes up to S$3,000 toward those three days — though actual ICU rates can be much higher.

4. Cancer Treatments

This is one of MediShield Life’s strongest benefits. It covers:

  • Chemotherapy — up to S$3,000 per month
  • Radiotherapy — up to S$2,600 per month
  • Immunotherapy and targeted therapy — covered under the outpatient treatment benefit at approved cancer centres

Cancer treatment costs in Singapore can run S$10,000–S$30,000 per month for advanced cases. MediShield Life’s limit is a meaningful contribution, but you will still face significant out-of-pocket costs on a MediShield Life-only policy.

5. Dialysis

Outpatient dialysis is covered at up to S$1,400 per month at approved centres. Dialysis is a lifelong ongoing treatment for kidney failure patients — this benefit can be extraordinarily valuable for those who need it.

6. Psychiatric Inpatient Care

Mental health hospitalisation is covered up to 60 days per year for inpatient psychiatric treatment. This reflects growing recognition of mental health needs.

7. Pre- and Post-Hospitalisation Treatment

MediShield Life also covers outpatient treatments directly linked to an inpatient admission:

  • Up to 3 days before admission (pre-hospitalisation)
  • Up to 90 days after discharge (post-hospitalisation)

This includes follow-up specialist consultations, physiotherapy, and prescribed medications related to the original admission — but only if connected to that specific inpatient episode.

8. Pregnancy and Delivery Complications

Serious complications of pregnancy that require inpatient care — such as ectopic pregnancy, severe preeclampsia, or emergency C-sections — are covered at B2/C ward rates. Routine maternity and normal delivery are not covered.

What MediShield Life Does NOT Cover

Understanding the exclusions is just as important as knowing what is covered. These gaps are the main reason most Singaporeans consider an Integrated Shield Plan.

Not Covered Why It Matters
Private hospital stays If you choose a private hospital, MediShield Life pays nothing. You need an ISP.
A-ward at public hospitals Even upgrading to A-ward in a public hospital is not covered. You foot the full bill.
Outpatient GP visits Regular GP consultations, polyclinic visits, or specialist outpatient appointments are excluded.
Dental treatment No dental coverage at all — not even emergency dental surgery under general anaesthesia.
Elective cosmetic procedures Any procedure classified as cosmetic is excluded, even if medically motivated.
Self-inflicted injuries Claims arising from intentional self-harm are not payable.
Congenital conditions (ISP only) MediShield Life covers congenital conditions, but private ISPs may exclude them.
Long-term nursing home care Residential nursing home stays are covered under CareShield Life — a separate scheme.

Radiotherapy S$2,600 per month

Deductible — What You Pay First

Before MediShield Life pays anything, you must cover the deductible yourself. In 2026, the deductible is:

  • S$3,500 per policy year for insured persons aged under 80
  • S$4,000 per policy year for insured persons aged 80 and above

The deductible is waived if you were already hospitalised and paid it earlier in the same policy year. If you have two hospitalisations in one year, you only pay the deductible once.

Co-insurance — What You Pay After the Deductible

After the deductible is met, MediShield Life pays 90% of the remaining claimable amount. You pay the remaining 10% co-insurance. For a S$20,000 bill (after deductible), that is S$2,000 out of pocket.

The co-insurance is capped in practice because of the annual claim limit — but for most Singaporeans on MediShield Life alone, a major surgery can still result in S$5,000–S$8,000 of personal expenses.

How Much Will You Still Pay Out of Pocket?

Let’s work through a real example. Say you have a knee replacement surgery at a subsidised B2 ward in a public hospital. The total bill is S$22,000.

  1. Step 1 — Deductible: You pay the first S$3,500 yourself
  2. Step 2 — Claimable remainder: S$22,000 – S$3,500 = S$18,500 is eligible for MediShield Life
  3. Step 3 — Claim limit check: S$18,500 is within the surgical claim limit, so the full amount is eligible
  4. Step 4 — Co-insurance: You pay 10% of S$18,500 = S$1,850
  5. Step 5 — MediShield Life pays: S$16,650
  6. Your total out of pocket: S$3,500 + S$1,850 = S$5,350

That is for a B2 public hospital ward. For a private hospital, MediShield Life pays zero — the entire S$22,000+ falls on you.

This is why many Singaporeans add an Integrated Shield Plan — to cover the deductible, co-insurance, and private hospital access. See our integrated shield plan comparison 2026 for a side-by-side of all 7 ISPs.

How to Fill the Coverage Gaps

MediShield Life gives you a solid base — but there are three main gaps that leave most Singaporeans exposed. Here is how to address each one.

Gap 1: Private Hospital Access

If you want to stay in a private hospital or choose your own specialist, you need an Integrated Shield Plan (ISP). All seven MOH-approved ISPs automatically include MediShield Life as their base — you do not need a separate MediShield Life policy. The ISP tops up the coverage to your chosen ward class.

ISP premiums start from around S$30 per month for a 30-year-old. Part of the premium can be paid from Medisave. See our guide on MOH integrated shield plan options for how to choose.

Gap 2: Out-of-Pocket Costs (Deductible + Co-insurance)

An ISP alone still leaves you with the deductible and 10% co-insurance. To reduce this further, you can add an ISP rider. Post-April 2026, new riders cannot cover the deductible — but they do cap your co-insurance at a maximum of S$6,000 per year. New rider premiums are roughly 30% cheaper than before due to the April 2026 rule changes.

For most people under 40 in good health, an ISP without a rider is a sensible balance. For a deeper look at rider rules, see our new ISP rider rules 2026 guide.

Gap 3: Long-Term and Outpatient Care

MediShield Life does not cover long-term disability or nursing home care. That is handled by CareShield Life — a separate mandatory scheme for severe disability support. For outpatient GP and specialist visits, you can use your Medisave (subject to limits) or consider a corporate insurance policy through your employer.

Should You Upgrade to an Integrated Shield Plan?

The answer depends on your personal situation. But here is a practical framework.

MediShield Life alone is sufficient if:

  • You are comfortable in B2/C ward at a public hospital
  • You have significant savings to cover the deductible and co-insurance
  • You have employer group insurance covering private hospital access

An ISP makes sense if:

  • You want the option of private hospitals or A-ward without catastrophic bills
  • You have dependants and want to protect the family from financial shock
  • You are self-employed with no employer cover as a safety net
  • You have complex health needs or want choice over which specialist you see

Frequently Asked Questions

What does MediShield Life cover in Singapore?

MediShield Life covers inpatient hospitalisation at subsidised B2/C-ward public hospitals, day surgery, ICU stays, cancer treatments (chemo up to S$3,000/month, radiotherapy up to S$2,600/month), dialysis (S$1,400/month), psychiatric inpatient care (up to 60 days/year), and pre/post-hospitalisation treatment linked to inpatient admissions. The annual claim limit is S$150,000. It does not cover private hospitals, A-ward, outpatient GP visits, dental care, or cosmetic procedures.

Does MediShield Life cover private hospitals?

No — MediShield Life does not cover private hospitals at all. It only pays for subsidised B2 and C-ward stays at public hospitals. If you stay in a private hospital, you receive nothing from MediShield Life. To access private hospital coverage, you need an Integrated Shield Plan (ISP) from one of the seven MOH-approved insurers. ISP premiums start from around S$30 per month for a 30-year-old non-smoker.

What is the MediShield Life deductible in 2026?

The MediShield Life deductible in 2026 is S$3,500 per policy year for insured persons aged under 80, and S$4,000 per policy year for those aged 80 and above. You pay the deductible yourself before MediShield Life contributes anything to a claim. The good news: if you are hospitalised more than once in the same policy year, you only pay the deductible once per year, not per admission.

Does MediShield Life cover cancer treatment?

Yes — MediShield Life covers cancer treatment, including chemotherapy (up to S$3,000 per month), radiotherapy (up to S$2,600 per month), and certain targeted therapies at approved cancer treatment centres. However, actual cancer treatment costs in Singapore can reach S$10,000–S$30,000 per month for advanced cases. MediShield Life’s limits help, but you may still face significant out-of-pocket costs, especially without an ISP or rider.

Can I use MediShield Life at a private hospital?

No — MediShield Life cannot be used at private hospitals. The scheme only pays for subsidised public hospital stays at B2 and C-ward. To access private hospital coverage, you need an Integrated Shield Plan. The ISP is built on top of MediShield Life — you do not get a separate MediShield Life claim. The ISP handles the combined claim and pays out according to your chosen plan tier.

Does MediShield Life cover pre-existing conditions?

Yes — MediShield Life covers pre-existing conditions. The scheme cannot exclude you for health reasons. This is one of its strongest features. However, if you add an Integrated Shield Plan (ISP) on top, the ISP insurer may exclude pre-existing conditions or charge higher premiums (loading). Getting an ISP when you are young and healthy avoids exclusions and loading — insurers can only apply them at underwriting, not after your policy is in force.

What is the difference between MediShield Life and an Integrated Shield Plan?

MediShield Life is the compulsory base scheme — it covers subsidised B2/C-ward public hospital stays. An Integrated Shield Plan (ISP) is an optional private add-on that extends coverage to A-ward or private hospitals. The ISP includes MediShield Life within it — the two are integrated into a single policy. You pay one combined premium; the insurer handles the MediShield Life portion automatically. The key difference is ward class, claim limits, and out-of-pocket exposure.

How much does MediShield Life cost per year in 2026?

MediShield Life premiums in 2026 range from around S$100 per year for young adults (age 21–25) to over S$1,700 per year for those aged 65–70 before subsidies. The premiums are automatically deducted from your Medisave account — you do not need to pay cash. The government provides premium subsidies for lower and middle-income Singaporeans, and the Pioneer and Merdeka Generation packages provide additional rebates for older cohorts.
